Short answer: N-Acetyl Cysteine (NAC) is one of the most evidence-backed supplements available, with the odd distinction of having extremely strong evidence in specific clinical applications (acetaminophen overdose antidote, respiratory conditions, OCD, addiction) and surprisingly limited evidence for the general wellness uses it’s most commonly sold for. It is the precursor to glutathione (the body’s master antioxidant) and reliably raises cellular glutathione in most people. The most evidence-backed general uses are: respiratory mucus clearance, OCD symptom reduction, and as part of the GlyNAC longevity protocol.

What NAC is and why it matters

N-Acetyl Cysteine is an acetylated form of the amino acid L-cysteine. Cysteine is the rate-limiting precursor for glutathione synthesis โ€” meaning your body’s glutathione production is limited by how much cysteine is available. NAC is essentially a more bioavailable cysteine delivery system. It has a long history in medicine: it’s been on the WHO Essential Medicines List since 1987 as the antidote for acetaminophen (paracetamol) overdose, where it works by rapidly restoring liver glutathione levels. The evidence for general supplementation

Respiratory health

NAC thins mucus by breaking disulfide bonds in mucoproteins โ€” this is a direct chemical effect, not a systemic one. A 2000 European Respiratory Journal meta-analysis of 39 trials found oral NAC (600mg/day) significantly reduced exacerbations and sick days in patients with chronic bronchitis vs placebo. This is one of NAC’s most robust clinical applications.

OCD and compulsive behaviour

NAC modulates glutamate, an excitatory neurotransmitter involved in obsessive-compulsive and addictive patterns. A 2012 Journal of Clinical Psychiatry RCT found NAC at 2,400mg/day significantly reduced OCD symptoms vs placebo over 12 weeks. Multiple RCTs show benefit in pathological gambling, trichotillomania (hair pulling), and addiction. The glutamate normalisation mechanism appears distinct from glutathione effects.

GlyNAC โ€” the longevity protocol

A 2022 Clinical and Translational Medicine RCT combined NAC with glycine (glycine provides the second rate-limiting precursor for glutathione, glutamate being the third). GlyNAC supplementation corrected glutathione deficiency in older adults and produced remarkable improvements in muscle strength, gait speed, cognitive measures, and mitochondrial function over 16 weeks. This is the most exciting recent NAC application and is increasingly recommended as a longevity supplement combination.

Important: NAC and FDA regulatory status

In 2020, the FDA issued guidance stating NAC is excluded from the dietary supplement definition because it was approved as a drug (as a mucolytic) before being marketed as a supplement. This led to Amazon removing NAC products. As of 2026, FDA has not taken enforcement action and NAC remains widely available, but the regulatory status is technically ambiguous.
