Does Ashwagandha Lower Cortisol? What the Clinical Evidence Shows
Short answer: Yes โ in people with elevated cortisol from chronic stress, KSM-66 ashwagandha at 300-600mg daily produces significant cortisol reductions of 14-32% within 8 weeks across multiple double-blind RCTs. The effect is real, reproducible, and clinically meaningful. It does not appear to suppress cortisol below normal levels in healthy people who aren’t stressed.
The RCT evidence for cortisol reduction
The most rigorous evidence comes from a 2012 Indian Journal of Psychological Medicine RCT (64 adults with chronic stress, 60 days, KSM-66 300mg twice daily vs placebo). Result: serum cortisol reduced by 27.9% vs 7.9% in placebo (p<0.0001). The same study showed significant reductions in all four measures of stress: PSS scale, GHQ-28, Oxford Happiness Questionnaire, and cortisol. A 2019 Medicine study (60 adults, 8 weeks, 240mg Sensoril ashwagandha) confirmed morning cortisol reduced by 23% vs 4% placebo. A 2021 Medicine study using KSM-66 at 600mg found 30.5% cortisol reduction.
The mechanism: why it works
Ashwagandha’s active compounds โ withanolides โ are believed to modulate the HPA (hypothalamic-pituitary-adrenal) axis. Under chronic stress, the HPA axis becomes dysregulated, producing persistently elevated cortisol. Withanolides appear to reduce the sensitivity of glucocorticoid receptors to stress signals, normalising the cortisol feedback loop rather than blocking cortisol production entirely. This explains why ashwagandha seems to reduce elevated cortisol but not suppress it in non-stressed individuals โ it’s regulatory, not suppressive.
Critical: form matters enormously
Only KSM-66 or Sensoril ashwagandha have robust RCT evidence for cortisol reduction. Generic ashwagandha root powder without standardisation has inconsistent withanolide content and may not produce the same results. Always check the label for these specific extract names.
What the evidence does NOT support
Ashwagandha is sometimes marketed as a testosterone booster. While one RCT (Wankhede 2015) showed testosterone increased significantly in men doing resistance training who took KSM-66, this is likely secondary to cortisol reduction โ high cortisol suppresses testosterone production, so reducing cortisol allows testosterone to recover. Ashwagandha is not a direct testosterone stimulant in the way that LH (luteinising hormone) agonists work.
Dose and timing
Effective doses in RCTs: 240-600mg standardised extract daily. KSM-66 studies typically use 300mg twice daily (with meals). Sensoril studies use 125-250mg twice daily (Sensoril is more concentrated). Timing: with food to improve absorption. Effects build over 4-8 weeks โ don’t expect acute cortisol reduction from a single dose.
