Is Creatine Safe for Kidneys? What the Science Actually Shows
Short answer: Creatine monohydrate at 3-5g/day is safe for kidneys in healthy adults with normal renal function. This is one of the most thoroughly examined safety questions in supplement science. A 2016 Journal of the International Society of Sports Nutrition systematic review of all available studies found no evidence of kidney damage in healthy populations at standard doses over periods up to 5 years. The confusion comes from creatine’s effect on serum creatinine โ a standard kidney function marker โ which rises with creatine supplementation but does not indicate actual kidney damage.
Why creatine raises creatinine (and why this doesn’t mean kidney damage)
Creatinine is a metabolic waste product of creatine metabolism, filtered and excreted by the kidneys. It’s used as a marker of kidney filtration rate (GFR) because healthy kidneys efficiently clear it. When creatine supplementation is ongoing, more creatine is metabolised, producing more creatinine. This increases serum creatinine and reduces estimated GFR, making kidney function appear impaired on standard blood tests. However, true GFR (measured directly) is unaffected. This means creatine creates a false positive on kidney screening tests โ a real problem for people who supplement creatine and then have a routine blood test, because their doctor may incorrectly interpret the elevated creatinine as kidney dysfunction.
The evidence for long-term kidney safety
Poortmans & Francaux (1999) followed athletes supplementing creatine (up to 5 years) and found normal kidney function by all direct measures including GFR, albumin excretion, and tubular function. A 2008 International Journal of Sport Nutrition study found no kidney damage markers in football players after 5 weeks of creatine loading. The 2016 systematic review examined all case reports of alleged creatine-related kidney damage โ in every case, the individual either had pre-existing kidney disease, was consuming excessive doses, or had confounding factors (such as concurrent NSAID use) that better explained the kidney impairment.
Who should exercise caution
People with pre-existing chronic kidney disease (CKD) should consult a nephrologist before using creatine โ not because creatine causes damage, but because elevated creatinine makes monitoring difficult, and any additional metabolic load on compromised kidneys requires medical oversight. People taking NSAIDs regularly should also use caution, as NSAID nephrotoxicity is a genuine risk that could be confounded with creatine use.
